The complex can be used by every family doctor, serve as a reliable means of early diagnosis of cardiovascular diseases and control the therapy being carried out, be an effective means of reducing the mortality of the population from cardiovascular diseases.

Dear Vic! The well-known C. method involves conducting a test of reactive hyperemia and,
ReplyDeleteusing ultrasound equipment, measuring the degree of increase in the diameter of the
brachial artery under the influence of an additional amount of NO released by the
endothelium, compared with the diameter in the initial section of the test. The degree
of increase in the diameter of the artery determines the function of the patient's
endothelium.
However, the C. method gives a large error in estimating the FE, since it does not
neutralize the influence of many interfering factors. In addition, the C. method
requires expensive ultrasound equipment, is methodologically complex and requires
highly qualified specialists. The C. method cannot be used for screening monitoring
of the state of the endothelium of the population, which is extremely necessary for the
early diagnosis of cardiovascular diseases.
In the proposed complex, using a two-channel tetrapolar rheograph, rheograms of the
patient's shoulders are recorded, a test of reactive hyperemia is performed, and the
degree of increase in the diameter of the artery is also measured compared to the
diameter in the initial section of the test.
The diameter of the artery depends on its elasticity and transmural pressure in it. To
obtain information about the function of the patient's endothelium in terms of the
degree of change in the diameter of the artery, it is necessary to exclude the influence
of a possible change in transmural pressure in the artery during the test on its diameter
and exclude the influence of a possible change in the tone of the vessel under the
influence of the system of central regulation of tone. This is done in the proposed
complex, but not done in the C. method.
In addition, endothelial local regulation of vascular tone is always carried out against
the background of central regulation of tone, making the necessary correction for each
specific region. It is clear that from patient to patient, from study to study, the
contribution of the central regulation of tone to the vascular tone of a particular region
will change. This will necessitate a change in the endothelial contribution, by
changing the amount of NO released by the endothelium. This means that the amount
of reserve NO will change, which can be used by the endothelium to reduce vascular
tone in the test of reactive hyperemia. This means that the degree of change in the
diameter of the vessel in the test of reactive hyperemia will depend on the base
amount of NO that the endothelium released in the background area of the test. This
effect is not taken into account in the C. method, which leads to a significant
additional error in the assessment of endothelial function, and it is completely
neutralized in the proposed complex.
There are a number of factors that are not taken into account in the C. method and
lead to additional errors in the assessment of the endothelium and which are
neutralized in the proposed complex.
The proposed complex is cheap, does not require highly qualified operators, gives a
small error in PE estimation and is suitable for population screening.
The complex works with a computer, and the cost of its additional hardware
(rheograph) can be significantly reduced with large volumes.
